About the role

You will work alongside a multidisciplinary internal team and external agencies, to achieve the best transition and accommodation outcomes for individual students.

You will work with learners to assess skills and aspirations to meet their outcomes beyond college. This could range from securing appropriate accommodation, education, volunteering and/or leisure opportunities.

The ability to network and liaise effectively with external agencies is essential.

This is a full time permanent position based at our beautiful Ullenwood campus with visits to other National Star locations on occasions.

Some key responsibilities of the role include but are not limited to:

*To effectively research key information advice and guidance in line with college and the National Frameworks relating to transition.

*To work with a caseload of individual students, providing transition information, advice and guidance in line with national frameworks.

*To liaise with families and carers of a selected group students and their families from baseline and at pivotal points to the end of their placement at NSC.

*To lead on the creation of a personalised and aspirational transition plan throughout their learner journey for a selected group of Year 1 students.

*To develop and maintain the IAG information and resource areas including the LAC power app and provide information in accessible formats to meet the needs of individual learners.
